HOLLYWOOD, FL - November 2, 2004 - VISTA announced today it will offer several Medicare Advantage HMO plans in 2005, including a new plan in Palm Beach County, to Medicare beneficiaries in South Florida beginning in mid November. Plans will be offered in Miami-Dade, Broward and Palm Beach counties and will have a January 1, 2005 effective date. Each of the plans feature no plan premium, low copayments and low out-of-pocket costs in addition to free exercise classes. Some plans include a prescription drug benefit, free transportation for physician office visits and a Complementary Alternative Medicine (CAM) network with discounts for acupuncture, at home massage therapy, yoga, tai chi and pilates classes and many more services. *The benefits are pending approval from the Centers for Medicare & Medicaid Services (CMS). "The introduction of VISTA's Choice Plan in Palm Beach County is an important step in our continued commitment to provide consumers with more choices and affordable health benefit plans that help minimize their out-of-pocket expenses," said Steve Russell, vice president of product development for VISTA. "This new product solidifies our position in the Medicare marketplace and is aligned with our strategy to increase Medicare membership in Palm Beach." VISTA Choice Plan - Miami-Dade, Broward, and for the first time in Palm Beach County The plans include no copayments for PCP and specialist visits, inpatient hospitals stays, services at ambulatory surgical centers, outpatient rehabilitative services (occupational, physical and speech therapy), free standing radiology centers and emergency room visits. There is a $50 a month over-the-counter benefit with more than 60 items to choose from. The plan also includes a $250 annual benefit for out-of-the area physician visits that can be used when traveling outside VISTA's service area. Free roundtrip transportation is provided for up to four physician visits a month. Additional services include dental and vision. The plan does not include a prescription drug benefit. VISTA Platinum Medicare Plan - Miami-Dade Features include no plan premium and no copayments for PCP and specialist visits. There is a four-tier prescription drug benefit $0/$10/$30/20% co-insurance. The Tier 1 benefit is unlimited. Tiers 2-4 have a maximum benefit of $275/month with a rollover up to three months per calendar quarter ($825). The copayment for outpatient hospital services is $25-$50 for a non-surgical procedure and $100 for a surgical procedure. There is no copayment for inpatient hospital stays, ambulatory surgical centers and freestanding radiology centers. There is no copayment for outpatient rehabilitation services (occupational, physical and speech therapy) when performed at a facility other than a hospital; a $25 copayment applies when services are rendered at a hospital. An emergency room visit is $50 which is waived if admitted to the hospital within 24 hours. Additional services include dental, vision, fitness/exercise classes, Complementary Alternative Medicine and out-of-area physician visit. VISTA Platinum Select Medicare Plan - Broward Features include no plan premium and no copayment for PCP. The specialist visit copayment is $10. There is a four-tier prescription drug benefit $0/$20/$40/20% co-insurance. The Tier 1 benefit is unlimited. Tiers 2-4 have a maximum benefit of $250/month with a rollover up to three months per calendar quarter ($750). The copayment for outpatient hospital services is $25-$100 for a non-surgical procedure and $100 for a surgical procedure. The inpatient hospital benefit is $50/day (days 1-5), per admission. The copayment at an ambulatory surgical center is $25 and $0-$25 at a freestanding radiology center. There is no copayment for outpatient rehabilitation services (occupational, physical and speech therapy) when performed at a facility other than a hospital; a $25 copayment applies when services are rendered at a hospital. An emergency room visit is $50 which is waived if admitted to the hospital within 24 hours. Additional services include dental, vision, fitness/exercise classes, Complementary Alternative Medicine and out-of-area physician visit. VISTA Prime Plan - Palm Beach Features include no plan premium and no copayment for PCP. The specialist visit copayment is $20. There is a four-tier prescription drug benefit $0/$17/$50/20% co-insurance. The Tier 1 benefit is unlimited. Tiers 2-4 have a maximum benefit of $275/month with a rollover up to three months per calendar quarter ($825). The copayment for outpatient hospital services is $25-$100 for a non-surgical procedure and $250 for a surgical procedure. There is no copayment for an inpatient hospital stay or services at an ambulatory surgical center. The copayment at a free standing radiology center is $0-$25.
